Types of Facilities:Our team care for patients across many levels of care. Below are descriptions of the most common types.Subacute RehabilitationSome nursing homes may provide skilled nursing care after an injury or hospital stay. They also offer different kinds of therapies to help a person recover after and illness or surgery. Patients typically stay for a short period of time and then transition back home or to an assisted living or custodial care facility.Assisted LivingThis group living arrangement provides help with activities of daily living such as eating, bathing,using the bathroom, taking medicine, and getting to appointments as needed. Residents often live in their own room or apartment within a building and have some or all of their meals together. Nursing Home/Long-Term CustodialNursing homes provide 24-hour care to people who cannot take care of themselves due to physical,emotional, or mental problems. A wide range of nursing care, personal care, and medical services are included, such as dressing, bathing, and using the bathroom. HospiceHospice care is a coordinated program of palliative and supportive services. It provides physical, psychological, social and spiritual care for terminally ill patients and their families.
