The Extended Care Team at Dreyer is dedicated to improving patient’s quality of care in extended care settings through on-site visits, family involvement, coordination of specialty care, continuity across various care settings, and the use of our electronic medical record.
Extended Care Team Services

Types of Facilities
Our team care for patients across many levels of care. Below are descriptions of the most common types.
- Subacute Rehabilitation
Some nursing homes may provide skilled nursing care after an injury or hospital stay. They also offer different kinds of therapies to help a person recover after and illness or surgery. Patients typically stay for a short period of time and then transition back home or to an assisted living or custodial care facility.
- Nursing Home/Long-Term Custodial
Nursing homes provide 24-hour care to people who cannot take care of themselves due to physical, emotional, or mental problems. A wide range of nursing care, personal care, and medical services are included, such as dressing, bathing, and using the bathroom.
- Palliative Care
Palliative care is focused on providing patients with relief from the symptoms, pain, and stress of a serious illness—whatever the diagnosis. The goal is to improve quality of life for both the patient and the family at any stage of the patient’s illness.
